SUPPORTING YOU TO STAY HEALTHY AND HAPPY AT HOME

Support at home to keep you happy and healthy

As a specialist home care provider, our aim is to keep you healthy and happy at home as long as possible.

Our approach focuses on wellness. By supporting you to retain existing skills and/or regain skills for everyday tasks, we can help you maintain an independent and connected life from the comfort and safety of your own home.

Our in-home services are individually tailored, taking into account your personal choices and your short and long-term care requirements. We assign each client a dedicated Coordinator so they get to know what’s important to you and you can trust them to be there for you when needed.

Whether you have a Home Care Package, Commonwealth Home Support Program (CHSP) funding or wish to pay for services privately, we are here to support you.  We are also fully prepared for the transition of Home Care Packages to the Support at Home program later in the year and your Coordinator will be there by your side to ensure everything is taken care of.

Aged care funding is hard to navigate and with the Support at Home program coming soon, you may need someone to help you understand all the options and how to access the support you or your loved ones need. Call us on 9319 0900 and we can help you navigate the system, get the best outcomes for your funding and let you know what private services are available while you wait for assessment or package approval. We also run community events on subjects such as demystifying aged care and falls prevention.

Our In-home services

Cleaning and laundry

We can assist with vacuuming, mopping and cleaning of bathrooms and toilets. We can also include dusting and help with laundry tasks. We encourage you to participate as much as possible.

Shopping

We can accompany you to local shopping centres to carry out essential shopping tasks including banking and paying bills. Alternatively we can provide a support worker to do the shopping for you.

Personal care

We offer assistance with showering/bathing, dressing and personal grooming, and help you prepare for the day. The level of assistance that may be required with daily self-care will be discussed with you to retain a reasonable level of independence.

Medication administration

This service involves administering your medication or prompting you to take your medications at specific times of the day.

Allied health  – Occupational therapy and physiotherapy

We provide home modifications, home exercise programs, falls prevention strategies, rehabilitation, prescription of walking aids and more to keep you healthy, independent and safe.

Meal preparation

Our support workers are trained in food safety, meal preparation and food hygiene. We can prepare healthy meals for you if you are unable to do this task independently. Diet preferences and healthy choices will be considered and we will encourage you to have maximum input into dietary requirements and the meals you would like.

Nursing services

Nurses can assist you after hospitalisation or as an ongoing service through a Home Care Package. Our services include clinical assessment, wound care and catheter care, scheduled medication administration, and wellness checks.

Social support

Our one-on-one support allows you to participate in outings for specialised shopping and to access the wider community. This is particularly important if there is a feeling of social isolation.

Keeping safe

To help keep you safe, we can help minimise environmental health and safety hazards in your home and garden. This may involve minor maintenance or repair work that doesn’t require a licensed tradesperson. Examples include:

  • Replacing light bulbs
  • Installing batteries in smoke and security alarms
  • Cleaning the gutter, fans, and external and internal windows
  • High indoor cleaning, such as the tops of cupboards and fridges
  • Some minor home maintenance, such as cupboard door adjustments
  • Pruning, lawn mowing or weeding where a safety hazard exists

In-home respite

We provide relief for carers by offering in-home respite care. If there is a carer who assists with your ongoing in-home care, they may be eligible for this service.
