Community Hubs

There is an evolving movement around the globe which focuses on new and innovative ways to address many challenges faced by an ageing population. This new hub way of thinking focuses on building community connection within local neighborhoods to increase the health outcomes and general wellbeing for older people living in their local area.

A New Model for Aged Care

Melville Cares is at an important phase on its journey to creating a new way of working with and alongside older people.

The new model of Community Hubs will link in with our existing community aged care work to find state-of-the-art practices that benefit health outcomes and wellbeing for our clients and people in the community, including our current volunteers.

Hub Development

We are currently in the process of evolving our Community Hubs in partnership with members from the Mt Pleasant Bowling Club, Attwell Gallery and the Potters Group, Alchera Living and Saint David’s Anglican Church in Applecross.

The people belonging to these groups will have the opportunity to direct how these hubs develop, and Melville Cares will be there to assist.

Further Information

Melville Cares also believes that based on the experiences of COVID19 there needs to be a clear focus on enabling and empowering older people with technology solutions.

How Do I Access These Services?

If you are seeking aged care support, our services are accessible through the following funding options. If you are seeking support as veteran, please visit our Veterans’ Home Care page for further information.

Funding Options

There are three options available to you to fund our services:


Commonwealth Home Support Programme (CHSP)
Access full or part government funding

Home Care Package (HCP) Levels 1 – 4
Access full or part government funding

Fully Self-fund

Commonwealth Home Support Programme (CHSP)

We are an approved provider of the government’s Commonwealth Home Support Programme (CHSP).

The CHSP is designed to provide funding support to those with low level care needs. If you require one or two services to help you stay living at home, this is probably your best option.

To find out if you’re eligible for the CHSP and to apply for an assessment of your care requirements, visit My Aged Care online or call 1800 200 422.

Your assessment will determine the level of funding you can receive and any out-of-pocket expenses.

Home Care Package (HCP) Level 1 – 4

We are an approved provider of the government’s Home Care Packages (HCP) programme Levels 1 – 4.

If your care and support needs are complex and ongoing, you could be eligible for a HCP.

The packages are available in four levels, ranging from more basic care (Level 1) through to higher care (Level 4). The packages are designed to meet your ongoing and increasing care needs to help you remain in your own home.

All services are delivered in accordance with the Commonwealth Consumer Directed Care (CDC) guidelines for Home Care Packages.

Your assessment will determine the level of funding you can receive and any out-of-pocket expenses.

Fully Self-fund

If you’re not eligible for funding support through the Commonwealth Home Support Programme or a Home Care Package, you have the option to self-fund.

Self-funding has the advantages of no waiting lists, no government assessments and less paperwork.

It also gives you complete flexibility to scale up or scale down your services as required.
