Restorative care

If you are eligible for the Restorative Care Pathway funding, you can access our services without the need for or use of Support at Home funding.

These services can help you get back on your feet after a fall, manage a change in mobility, or enable you to be discharged from a long term hospital stay back into your own home rather than residential care. We can move quickly so you are not kept waiting too long.

You can receive up to 16 weeks of intensive allied health support, such as Physiotherapy and Occupational Therapy to help restore or maintain function.

Meanwhile, our team can identify what services may assist you to continue living independently once the period of intensive support has ended.
