End of life care

We know most people would prefer to stay at home for the last part of their lives, but very few are able to. The Government’s End of Life Care Pathway has been created to provide additional funding that enables people to access additional home care services, so they can remain where they are most comfortable.    

We work closely with Palliative Care WA and Edith Cowan University on resources to support advanced care planning and we host frequent events and 1:1 sessions to help people through the process.  We have also partnered with Critical Info to offer access to their online planning resources.
